Supervision Meetings
Please use the following log to document your independent hours collected weekly.
Prior to the face-to-face supervision meeting, please have the following log filled out. This will assist us in determining how long the supervision period will last.

Notes
The BACB Experience Supervision Form, which can be found on the BACB website and will be filled out each supervision period.
This form will be kept by the supervisor and the supervisee for seven years (in case of an audit).
These forms will NOT be sent to the BACB to sign-up for the exam, but the Experience Verification Form will be sent to verify completed hours.
Supervision notes will be kept by both supervisor and supervisee. The notes are a summary of the meeting as well as any feedback provided to the supervisee.
